Young People’s Transitions Practitioner
The Young People’s Transitions Practitioner offers person-centred support for young people, aged 14-21 years, who have a learning disability and/or autism and are currently/have previously been under the support of local Child and Adolescent Mental Health Services (CAMHS), Kooth, MIND (Be Kind to Your Mind, Learnwell or Mental Health Support Teams), Intensive Behavioural Support Service or Positive Behavioural Support Consultancy services, mental health youth Workers.
The service is available for residents of Hammersmith and Fulham, Royal Borough of Kensington and Chelsea (RBKC) and Westminster.
Our Senior Young People’s Transition Practitioner is Priyanka Kunwar for RBKC and our Young People’s Transition Practitioner are Rowan Wasfi for Westminster and Natasha Gurung for Hammersmith and Fulham.
The main aim of the service is to help young people think about their adult life as a whole, rather than simply thinking about what services they might be accessing post-18. The Young People’s Transitions Practitioner works with the young person to think about:
- Hopes and dreams for the future
- What is working well currently
- What is not working well currently
- What is important now
- What is important in the future
Alongside this the service aims to provide:
- Signposting and recommendations for suitable activities and services that are not already being accessed in relation to young person’s goals
- Identifying and mapping out links between professionals involved and ensuring young people and their families are aware of who is involved and what their role is
- Organising and promoting the use of handover meetings in order to ensure the young person’s needs will be met
- Attendance at meetings relevant to young person’s needs to ensure their preferences are being heard and taken into consideration
- Identifying unmet needs and investigating options to provide additional support for young people in relation to their goals
In the past year 100% of young people who have accessed the service felt listened to and more positive following service involvement. Additionally, 100% of parent/carers would recommend the service to other families going through the transition process.
