    We welcome: Parents/carers of young adults (18 plus) with learning disabilities and/or autism/neurodiversity Colleagues working across health and social care in Hammersmith & Fulham             Third sector organisations providing adult care and support.   The conference has three key aims:

    • To help deepen understanding of the transition process to adult social care and adult health services
    • To help deepen understanding of the support options available for young people and their families once they transition to adult services.
    • To help identify what we may need to do more of, less of, or differently, to better meet the aspirations of young people and their families.

Anastasia joined in 2019 and is an experienced health and social care manager with a history of working across the mental health, learning disabilities and digital healthcare sectors. She has an academic background in Clinical Psychology (B.Sc., M.Sc.) and more recently undertook a course in Healthcare Management and Leadership (PG.Dip.). Anastasia’s multilingual and multicultural background has supported her in working effectively with service users and families from different cultures and faiths, something very distinctive of London’s population. She will be overseeing operations of the MyLife and Play projects and will support the CEO in business development and quality assurance.
