Transitioning into the right care, at the right time in the right place

We welcome:
Parents/carers of young adults (18 plus) with learning disabilities and/or autism/neurodiversity
Colleagues working across health and social care in Hammersmith & Fulham
Third sector organisations providing adult care and support.
The conference has three key aims:
- To help deepen understanding of the transition process to adult social care and adult health services
- To help deepen understanding of the support options available for young people and their families once they transition to adult services.
- To help identify what we may need to do more of, less of, or differently, to better meet the aspirations of young people and their families.
Participants can choose to attend three workshops from a total of five.
